After you are discharged from the hospital, you must call us at 516-775-0493 to make a follow-up appointment for your newborn or child.

For newborns:

• About your Newborn brochure   A general information page on taking care of your newborn
•     Newborn Hearing Screen  All infants are now screened for hearing in the hospital. Learn more information on the process

•    Newborn Blood Screening After 24 hours of life, all newborns have a blood test. You have been supplied with a "pink" piece of paper.  Learn why we need that paper, and why it is so important.

Before you visit our office:

We will need the following documents read and an acknowledgement form filled out when you come into the office for the first time.  You can save time if you print them AT HOME, read, and sign the form, and bring them with you to the office.

• Registration forms 
• Financial Information 
• Privacy Policy
• Acknowledgement Form
• Insurance Information
• Immunization Records
• Photograph Authorization Form (optional)

In addition we need copies of the following (Due to HIPAA regulations)

• Parents Driver's License or Picture ID
• Parents Social Security Card
• Children's Social Security Card
